The School Nurse Data Committee requests your participation in this survey to measure health office outcomes, health office workforce, the percentage of students in Nebraska that have a chronic condition, and the completion of state mandated tasks. The survey questions are created to mirror the
National Association of School Nurses (NASN) Uniform Data Points. Additionally, this year we are including a supplemental Vision Care Survey.
By submitting information into this survey, you are giving consent for this information to be shared with NASN as part of their National School Health Data Set. No identifying information will be shared.
This survey can be completed at the district OR building level. If completing at the building level, please complete 1 survey for each building you cover.
Thank you for taking the time to complete this survey. If you are unable to answer every question, please complete the ones you can and leave the others blank. Results will be aggregated and published at the state level by the Data Committee and shared later this year.
