Thank you for your interest in the Arts For Life Best Performance Awards and Theatre Mask Awards. Please answer the following questions if you would like your theatre group's production(s) to be included in the 2026 year of judging (shows performed in the 2026 calendar year).
If possible, we ask that your group fill out 1 form for your entire calendar year of shows (all shows being performed by your group in 2026). If that is not possible you can always fill out another form when all information is known.
