2026 Individual Excellence Award Nomination Form

This award category is available to both nursing facility and assisted living members. The purpose of this award is to recognize an outstanding employee in a long-term care member facility that provides support to staff members who work directly with residents. They may include Social Workers, Environmental Staff, Activity Professionals, Dietary Staff, Business Office Managers, Marketing Admissions Coordinators, and all other ancillary services. (This award is not for nurses. If you wish to nominate a nurse, please use the CNA of the year, Nurse of the year. and DON of the year categories.)
The goal of this award is to honor outstanding employees who work in support of the delivery of care in their facilities.
