2026 Futures Award Nomination Form

Submission Deadline is Friday, April 10, 2026

This award category is available to both nursing facility and assisted living members. Anyone employed in long-term care for three years or less can be nominated.
The goal is to honor outstanding employees who are new to long-term care but have already demonstrated compassion, caring, and innovation in dealing with residents, families, and staff in the delivery of care.
