2026 Mutual of Wausau 4-H Club Grant Program Application

The purpose of the Mutual of Wausau 4-H Club Grant Program is to support 4-H Clubs in youth development and to promote civic and leadership responsibilities. Grants will be awarded in the amount of $400 per club. In order to be considered, your application must be submitted by April 1st. If you need to send additional information, please email it to info@mutualofwausau.com. Include the grant you are applying for and the name of the Club.

3.County where club is located.(Required.)
4.Number of members in club.(Required.)
5.List of Club activities for the last 2 years.(Required.)
6.Purpose for which you are requesting a grant.(Required.)
7.List fundraising activities for the project which the grant is requested.(Required.)
8.Number of members involved in project for which the grant is requested.(Required.)
9.Other fundraising activities that the Club is involved in.(Required.)
10.Insurance Agency, City of Agency, and Agent Name that referred this grant program to you.(Required.)
11.Any additional  information that you would like to add. (Required.)
