PharmD Early Assurance Program Application

Please fully complete the application to be considered for the PharmD Early Assurance Program at the University of Oklahoma College of Pharmacy. 2025-26 applications are due by April 15, 2026 of the senior year of high school.

2.Are you a U.S. citizen or U.S. permanent resident? This is a requirement for eligibility and the PEAP program. Applicants must be a US Citizen or Permanent Resident at the time of application.

4.Have you completed any AP/IB credit that you plan to receive college credit for?(Required.)
5.If you answered yes to the question above, please list the AP/IB course(s) you plan to take the test for and receive college credit. If you answered no, you may skip this question.
6.Have you taken any college courses while in high school?(Required.)
7.If you answered yes to the question above, please list the college/university attended and the course(s) you received college credit for. Include the course name and number, your grade, and number of credits for the course(s) (e.g. University of Oklahoma, ENG 1113 Composition I, A, 3 hours). If you answered no, you may skip this question.
8.If you have received grades for college courses, what is your current overall college GPA?
9.Please list your top 3 college choices after high school graduation.

13.Please enter your highest ACT composite score or combined SAT score. When you submit your ACT or SAT scores, they do not have to be official. You can email a copy of your personal score report to OUPharmD@ouhsc.edu.(Required.)
Please list the names and emails of the two references you plan to provide for your application. One must be a math or science teacher. Your other reference can be from a different teacher, coach, counselor, work supervisor, etc.

19.Please provide a personal statement as to why you want to be a pharmacist. Your statement must be a minimum of 300 words. You should use proper grammar, spelling, and punctuation. You may copy and paste from a Word or Google document if you wish, but you may not use any AI software, i.e. ChatGPT, etc., to generate your statement.(Required.)
