The Heart of Case Management Awards recognize case managers who make extraordinary impacts on the lives of injured employees. This prestigious national program honors Enlyte's case managers who go above and beyond their traditional duties, demonstrating exceptional commitment to improving outcomes for those they serve.

Our client awards offer you the opportunity to recognize case managers who have made exceptional contributions to your organization and the injured employees you serve. We value your perspective and believe you witness firsthand the difference that outstanding case management makes.

We now welcome nominations throughout the year! This ongoing submission process allows you to nominate outstanding case managers when their exceptional work is fresh in your mind. We'll send periodic reminders to encourage nominations and will make this form available throughout the year.

Winners are selected from over 2,000 case managers nationwide based on their remarkable contributions and outcomes. In addition to receiving a monetary reward, honorees will be celebrated during Heart of Case Management Week each fall with a public announcement and a virtual awards ceremony open to all employees and winners' nominating organizations.

How to Participate
We invite you to nominate case managers who have demonstrated:
  • Exceptional commitment to injured/ill employee care
  • Creative problem-solving that overcame significant barriers
  • Outstanding communication and collaboration with your team
  • Measurable positive outcomes for the injured/ill employees you serve
Recognition for Winners
Selected case managers will receive:
  • Monetary award recognition
  • Acknowledgment in a public announcement during Case Management Week in October
  • Special recognition during our exclusive virtual awards ceremony
Nomination Guidelines
  • Eligible cases must have been closed between October 1, 2025 and today's date
  • Please complete the nomination form below with specific examples of the case manager's exceptional service
