MSNA Hero Award Nomination Form

The MSNA Hero award recognizes an individual who stands out as a Hero to their peers, students and staff.

Entry Guidelines
  • Individuals must be nominated by any staff member within or outside the school district. Industry members are eligible to submit nominations. Self-nominations are not accepted.
  • The nominated member must be a school nutrition employee. Directors are not eligible for this award.
  • The nominator and nominee must both be SNA/MSNA members.
  • Multiple candidates may be nominated from each district.
  • Please note that question 5 is optional but highly encouraged. This question asks for one or two statements from others in the district regarding this nomination. If providing, please include the name and email address of those providing the statements.
  • Nominations must be submitted online by March 1, 2025 at 11:59 PM, CT.
Recognition
  • Certificate, Letter to Principal, Director and an Invitation to President’s Reception and Welcome Reception at Annual Conference.
  • Recognized for School Lunch Hero Day (May 2), being featured on the MSNA website, social media, and newsletter.
