Dear colleagues,

To facilitate collaboration in a field with multiple stakeholders, the ISN proposes to organize a cross-disciplinary network ("i3C") linking diverse groups working on chronic kidney disease of unknown origin (CKDu) throughout the world. As one concrete contribution, i3C will curate a list of studies designed to promote communication and collaboration among research groups and communities.

We invite you to share a basic outline of human studies in the following form. If you are limited in what details you can provide, then even answering the first 3 very basic questions of the survey will help to document study locations and provide context for colleagues working on diverse aspects of CDKu and agencies supporting research in this area.

We estimate that the entire survey will take 5-10 minutes.

Thank you for sharing this survey (https://www.surveymonkey.com/r/ISN_CKDU_survey) with your colleagues.

If you would like a PDF version of the survey in English please email ckdu_observatory@theisn.org

Thank you for your involvement and readiness to participate.
