2026-2027 MATEC Clinician Scholars Program Application

Disclaimer: The Clinician Scholars Program is funded through the Midwest AIDS Training + Education Center (MATEC), which is a federally-funded grant program under the Ryan White HIV/AIDS Program (Part F). All MATEC programming, including the Clinician Scholars Program, is contingent on continued funding. We commit to notifying applicants at the earliest point possible of any funding changes that might impact the continuation of this program.
We appreciate your interest in the MATEC Clinician Scholars Program. This form is your application and will take approximately 30 minutes to complete. In addition to this application, you must also submit a Statement of Workplace Endorsement and may choose to submit a Letter(s) of Recommendation. This application and all supporting documents must be submitted no later than close of business on June 15, 2026 to be considered for the 2026-2027 cohort of the MATEC Clinician Scholars Program.

  • Letter(s) of Recommendation are strongly encouraged, but not required. A letter of recommendation should be submitted by someone who knows your clinical capacity, either a current or recent supervisor, or a colleague familiar with you as a team member.
