Just fill out the information below and be sure to include your email. Once you submit your form, please come to our District Headquarters, 5700 Almaden Expressway, San Jose, and receive your requested items. Office hours are Monday to Friday, 8 a.m. to 5 p.m.

*You must be a resident of Santa Clara County. One sign and bucket per household please, while supplies last.
