Ensure the next CHEX Conference reflects your priorities

Community Health Exchange (CHEX) is looking forward to next year’s (June 2015) national conference and as always we want your priorities to shape the event. To kick start our planning please share your initial thoughts via this short survey. We would be even happier to hear if you would like to participate in a short conference planning group.

The survey questions are only initial suggestions to provoke thought, for ease we have given some suggestions but please use the open text boxes to highlight your ideas.
