Request for Transportation Service
FOR SEPTEMBER 2013 TO JUNE 30, 2014 SCHOOL YEAR
Please complete one form for EACH student in your household for whom you are requesting transportation services. Please read the following guidelines.
DO NOT use this for for special needs transportation (as such transportation will be handled separately).
By providing the following information, parents/guardians understand and agree that any or all information may be shared with First Student Canada (school bus transportation services).
1. Students who reside in their home school catchment and live greater than 4.0 km for kg to grade 3 and greater than 4.8 km for grades 4 through 12, may apply for transportation (from the stop closest to their residence to school and back again) as per Ministry of Education funding guidelines.
2. For those who do not meet the criteria #1 distance, but reside in the school catchment area, courtesy passes may be granted as space permits, provided the bus does not deviate from its scheduled route to accommodate such courtesy riders.
3. Students must register and be on the roster of the bus or service will be refused.
4. Students who do not ride for a period of 3 weeks may be dropped from the roster and will have to reapply for service.
5. Please note that any bus run can be cancelled with a minimum two weeks notice if the number of eligible riders drops below the levels for Ministry funding.
6. Please note that the address on this application must match the address on file with the school. This application may be rejected if the addresses do not match.
7. This form is not to be completed for Special Needs Service. Please contact the Transportation Department for registration procedures.
8. Approval for ridership is determined on an individual student basis. Please understand that there is no provision for transportation to or from daycares or friends’ houses, etc.
