INSTRUCTIONS FOR COMPLETING THIS SURVEY
If possible, we ask that you provide the requested information on a site-by-site basis by completing a separate survey for each site. (A site may be designated by generic identifiers, such as “Site X” if that would be preferable.) If that is not possible, we would greatly appreciate receiving a separate survey for one or more representative sites and one with aggregate data for the company’s U.S. facilities.
“RCS” is an abbreviation for “respirable crystalline silica.”
When providing estimates of the annual operating costs and maintenance costs and time required for implementation of those OSHA-preferred measures, include capital items that will need to be acquired and eventually replaced, and provide their estimated life and total replacement costs (e.g., engineering and design, purchase, installation, down time). Please explain whether the engineering controls you have identified are based on existing technology and products, or something that would have to be developed (e.g., invented, scaled up, etc.).
