Instructions
This questionnaire asks for your opinion about your school and about your own learning. We want to know your thoughts and feelings so we can make your school an even better school for you. Please be honest and fair in your answers. Think about how things really are. No one in your school will know who said good or bad things about them. Your name will not be used on any reports about the people who work in your school. Your ID number is used only so we can find out information about you, like your gender and grade level. Then we can tell the adults at your school how, for example, girls or boys answered an item.
