The following survey is part of a study, funded by the California HealthCare Foundation, examining county-level experiences in implementing mandatory managed care for seniors and persons with disabilities (SPDs) covered by Medi-Cal, which began in June 2011. The goal of this project is to better understand the experiences of and range of activities used by local stakeholders (e.g., health plans, health care providers, advocacy organizations, and other community based organizations) in preparing for and implementing the transition to managed care. Therefore, the focus of this study is on health plans, health care providers, and other stakeholders. The experiences of beneficiaries will be documented and analyzed in a separate study.

Your survey responses will be anonymous and only reported in the aggregate. Information gathered will be used to better understand the experiences of those involved with the current SPD transition to managed care, as well as inform upcoming initiatives targeting children with special health care needs and dual eligible (Medicare-Medicaid) beneficiaries. In particular, we hope to identify effective activities used in counties that can be implemented more broadly to improve the experience of beneficiaries and health care providers in the transition to managed care.
