MAKING PROUD CHOICES SURVEY
INSTRUCTIONS
Please answer all of the questions in the survey as honestly as possible. On each page, scroll down and try to answer all of the questions on each page. Once you complete all of the questions on a page, click "Next" to move on.

The survey is about 100 questions long and will take you about 20 to 25 minutes to complete. Your answers are important to us, so please take your time and try to answer all of the questions.

If you have any questions about the survey or the meanings of words, please raise your hand and the facilitator will come over to you.

Some of the questions ask about things that happened in the past 3 months and some questions ask about things that may happen in the next 3 months. By the past 3 months, we mean what happened between January 2015 and now. When we are asking about the next 3 months we mean between now and July 2015.
CONFIDENTIALITY
Thank you for your help with this important study. It will help us understand what things are like for people your age today. Your answers are confidential and everything you say will be kept private. Please answer all questions as well as you can.

We want you to know that:

1.We hope that you will answer all the questions, but you may skip any questions you do not wish to answer.

2.The answers you give will never be identified as yours. Your responses will be combined with those of other people your age.
