You are invited to participate in a study of children’s motivations for behavior. This project will inform us about the usefulness of a particular survey instrument to measure these motivations. You have been asked to participate in the research because you are a PARENT of a OF A CHILD BETWEEN AGES 6 AND 16.

Anyone over the age of 18 who has a child between the ages of 6 and 16 is invited to participate; participation in this survey is ENTIRELY VOLUNTARY and you can refuse without penalty.

WHAT YOU WILL BE ASKED TO DO? You will answer 98 questions about your child’s personality and behaviors, which will take you about 15 minutes. You will need to complete it at once; you will not be able to save and return later.

WHAT ARE THE POSSIBLE DISCOMFORTS OR RISKS? There is a minimal possibility of mild psychological discomfort. You will be asked things that may make you uncomfortable, such as reflecting on your child’s tendencies and character traits. The answers you provide will be COMPLETELY ANONYMOUS, and YOUR NAME WILL NOT BE CONNECTED TO YOUR RESPONSES. Also, you may SKIP QUESTIONS if you do not feel comfortable answering, you may QUIT AT ANY TIME, and your decision will not affect your future relations with Sam Houston State University. If you do experience continuing discomfort, students of SHSU may contact the Counseling Center (936-294-1720), and other persons may locate counseling services in your area via the National Mental Health Self-Help directory (http://www.cdsdirectory.org/). If you feel that you are in a suicidal crisis, you may call the National Suicide Prevention Lifeline at 1-800-273-TALK (8255).

WHAT ARE THE BENEFITS? Some university students may receive research participation credits for participating in this study, which is at the discretion of your course instructor.

HOW WILL YOUR PRIVACY BE PROTECTED? Your answers will be COMPLETELY ANONYMOUS. The survey data will be downloaded from the Web server, saved in a password-protected file, and only associated researchers will have access to the data. Your survey responses will be kept confidential to the extent of the technology being used. SurveyMonkey collects IP addresses for respondents to surveys they host; however, the ability to connect your survey responses to your IP address has been DISABLED for this survey. That means that I will not be able to identify your responses. You should, however, keep in mind that answers to specific questions may make you more easily identifiable.
