Patient Experience and Feedback Survey

Dear Medicaid Member:

ICHP is asking our Medicaid Members (or the parents/guardians of our Medicaid Members) about their health care services. We want to know how well we are doing. We also want to know if there is anything we can do better. This survey will only take about 5 to 10 minutes. It is private. The only people who will see your answers are ICHP staff. Taking the survey will not affect you or your child's Medicaid benefits at all.

Please take a few minutes to take the survey. It will help us make health care services better.

Most people think about their doctor as the person who sees them when they’re sick and writes their prescriptions. When taking this survey, think about the last doctor's appointment you had for either you or your child. The doctor could be a:

(1) primary care doctor – this doctor has had many years of training after college. They either have an M.D. (medical doctor) or D.O. (doctor of osteopathy) after their name.

(2) a specialist is a doctor who is an expert in special health problems, like a doctor who delivers babies. They have an M.D. or D.O. after their name.

(3) a physician assistant – a person who has had almost the same amount of training as an M.D.. There are some things they can’t do, like do operations. They have a P.A. after their name.

(4) advance practice nurse (APN) – a nurse who has had several more years of training than a registered nurse (RN). An APN can write prescriptions and give more care than an RN.

(5) a psychiatrist – this is a doctor (M.D. or D.O.) who is expert in treating mental health problems.
