The Hartford Change AGEnts Initiative is an elite interprofessional network of over 3,000 individual’s nationwide who have all benefited from the John A. Hartford Foundation. The Hartford Change AGEnts Initiative leverages the Foundation’s powerful community to help its scholars and grantees learn from and support one another while they adopt, evaluate, and sustain changes in practice and service delivery that improve the health of older Americans and their families.

The Initiative strives to empower the Hartford community to translate knowledge and expertise into sustainable practice change through conferences, institutes, webinars and an Online Platform Community.

Through the Change AGEnts Online Platform Community, you can connect with an interprofesional group of leaders working to make practice change in various communities and settings across the country. Use the Open Forum to post a discussion thread, ask a question to your peers, get feedback or share an opportunity for involvement. Search the directory to find others with similar practice change interests, working in a complementary field or in a specific geographic location; direct message to learn more about how you can collaborate on work. Opportunities unique to the Change AGEnts will be posted and only accessible through the password protected Online Platform.
