Introduction
You are invited to take part in a research study which will look at the proportion of the general Australian community that follows a modified diet and the reasons why people choose to be on a particular diet. The study is being conducted by Dr. Louise Wienholt, A/Prof. Stephen Adelstein, and Ms. Stephanie Chu of the Department of Immunology at the Royal Prince Alfred Hospital

If you would like to participate in the survey, please answer the questions below. It will take about 5 minutes to complete the survey.
Costs
Participation in this study will not cost you anything, nor will you be paid.
Voluntary Participation
This study is entirely voluntary. You do not have to take part in it. By completing and submitting this survey, you are consenting to participate in it.
Confidentiality
All the information collected from you for the study will be treated confidentially, and only the researchers named above will have access to it. The study results may be presented at a conference or in a scientific publication, but individual participants will not be identifiable in such a presentation.
