Your feedback makes a difference! The Grand Vision recently concluded the final year of a three-year grant from the W.K. Kellogg Foundation. In order to evaluate the use of these funds, we need to measure change in the capacity of the Grand Vision Networks. This assessment should look familiar - it is the same one we have administered every year.
The assessment should take about 10 minutes to complete. Your responses will be anonymous, kept completely confidential and will only be used in aggregate to improve Grand Vision support and to report outcomes to the W.K. Kellogg Foundation.
After completing the survey, you will have the option of entering a drawing to win one $100 Downtown Traverse City Gift Certificate. If you choose to participate in the drawing, you will be asked to provide your name and contact information after you complete the survey, but it WILL NOT be tied to your responses. Your responses will remain anonymous. The winner will be announced the week of June 23rd.
The survey and drawing will close at 5:00 PM on Tuesday, June 17th.
