About the survey

This survey is part of a research study being undertaken by the Department of Food, Agricultural and Resource Economics at the University of Guelph with funding from the Ontario Ministry of Agriculture, Food and Rural Affairs (OMAFRA).

The study aims to better understand the challenges that farms and food businesses face in managing food safety and traceability. The results will help OMAFRA to improve the support services they provide to farms and food businesses in the province.

The survey will take about 30 minutes to complete. Participation is completely voluntary and you are free to stop completing the survey or to withdraw from the study at any time. In addition, you can skip any questions you do not wish to answer.

Individual responses to the survey will remain confidential to the researchers conducting the study. The results will only be reported in summary form, with responses stored securely with names and contact details removed. Note that, whilst we will store the survey data securely, we cannot guarantee the security of data transmitted over the Internet.

By returning the survey you will receive one entry to a prize draw for an iPad. The draw will be made on December 31, 2014. The odds of winning are around 1 in 500.
