We are collecting information about community-based innovations, along with existing activities or ongoing programs, that increase the use of clinical preventive services (CPS) (for example, immunizations, cancer screening, tobacco counseling) by older adults (ages 50 and over). To find out if a particular innovation or program qualifies for inclusion in this review, please answer the following short survey which we estimate will take you less than 3 minutes to complete.
