This 8-question survey is for Madison College faculty members involved in the Mentoring Program and should take a maximum of 10 minutes. Your input is valuable. This survey is non-confidential and your name will be required. Completion of this survey is required, along with the Faculty Mentor Stipend Request Form, in order to receive the $250 stipend (the stipend is available only to FT Faculty Mentors and PT Faculty Mentors & Mentees).
