2013 Annual Report |
State Council and Chapter Charters are maintained through the submission of this 2013 Annual Report. If the form is not received (complete) by January 31, 2014 your State Council or Chapter Charter may be at risk for suspension and assessment monies may be withheld. Since the report asks you about many different items, it is best to gather all the information you need before starting to input your data. (See PDF document with all the Annual Report questions.)
If you are a leader responsible for both a State Council and a Chapter, you need to complete an Annual Report for both organizations. Select "Both" on Question One of the survey to complete the reports for each organization.
Note: Submit the following to componentrelations@ena.org by dates shown (state councils/separately incorporated chapters only)
Dec 31 (2013) – change of address form – to be filed with IRS upon treasurer and/or president changing office
Jan 31 (2014) – 2014 budget; copy of current bylaws; copies of current polices
May 31 (2014) – copy of IRS Form 990, 990N, or 990EZ (for 2013 tax year)
