1. Overview

This survey is designed to obtain information from agencies and organizations about the number of employees (full or part-time) who are peer support providers, the number of volunteers, and the types of programs where they are used to provide support services.

For the purposes of this survey, a peer support provider, sometimes abbreviated as "peer provider," or "peer supporter," is someone who has lived experience with a mental health, addiction, trauma, or related condition who is in recovery and uses his/her lived experience to help others who have similar conditions.

Depending on the agency or program, a peer provider may or may not be paid for his/her services. All agencies and organizations are invited to respond to this survey, whether the peer support providers in their organizations are employees, contractors, or volunteers.

Please complete only one survey.

All information will be kept confidential and will be tabulated and reported without reference to any individual organization or its identifying characteristics.

Feel free to share and ask other organizations and agencies to complete this survey too! (A companion survey to gather data from peer providers is also available.)
