Passengers reserve seats below for round trip bus service from the University of Arizona campus in Tucson, Arizona, to Phoenix, Arizona, for those participating in the SOT High School events. Deadline: February 24, 2014.

Reserving space on this bus requires submission of a Liability Waiver. Parents or guardians must sign the waiver for passengers younger than 18. Please print The appropriate form, have the responsible party sign, and send to SOT Headquarters. Once signed, the form could also be scanned and sent back by email for students at Title 1 schools. Address information is below and on the form.

The fare for the bus is $5. Parents/passengers should mail a check for that amount made out to "Society of Toxicology" to the address below. Put the name of the registrant in the check memo field. Note: This fare will be waived for students who attend a designated Title 1 Schools but the liability waiver is still required.
