Women’s Centre for Health Matters are conducting this consultation to obtain the views of ACT women who have a disability who live and/or work in the ACT, aged 16 years and above, to find out:

1. How ACT women who have a disability understand and manage their health needs.

2. Which health services, supports and information they use and whether they experience barriers to that access.

3. What barriers they experience to maintaining good health and wellbeing.

We are asking carers (those in carer relationships with women with disabilities) to fill out the survey about the women with disability that you are caring for. This is to give women with a disability a voice, who might not otherwise get the opportunity.

Participation in this survey is voluntary. You may withdraw your participation at any time by exiting the survey or choosing not to submit the survey. If you consent to participate in this survey by completing and submitting the survey, we will use your answers for social research purposes.

This survey will take approximately 15-20 minutes to complete; however actual completion time will vary depending on the length of answers you would like to provide.
