Chapter Start-Up Form

Please use this two-page form to submit your interest in starting a UNA campus chapter. We are currently not starting community chapters at this time. Once we are, we will add a chapter start-up form for those specific details!

Please answer all questions. This form does not obligate you to anything - it lets the National Office know that you would like to speak with us to learn more about the process, requirements we have of chapters, and more so we can decide whether we can move forward!

UNA-USA chapters can only be located within the United States. Please be sure to check our chapter map to make sure there is not already a chapter near your area.
