Welcome to the Texas Compassionate Use (CUP) physician feedback survey.

The Texas Compassionate Use Program (CUP) is inviting you to participate in one or more surveys about the state’s Low-THC (medical cannabis) program. Your insights are valuable in helping us understand the experiences and perspectives of Compassionate Use Program participants. You are not required to take this survey. If you choose not to take it, your relationship with the program will not be affected in any way.

This survey is being conducted by a consultant contracted with DPS to gather data and provide recommendations about the program. The information will be used to improve the program and inform others, including legislators, program staff, policy makers and the public about the program’s impact, accessibility, and opportunities for improvement. If you choose to participate, please answer the following questions honestly and to the best of your ability.

If you have had different experiences with the CUP program over time, please answer each question based on your most recent applicable experience. You will have an opportunity to provide additional input at the end of the survey.

All responses are anonymous. We do not solicit or collect personally identifiable information.

For purposes of this survey, all references to ‘medical cannabis’ are intended to refer to ‘low-THC cannabis,’ as defined by Texas Occupations Code §169.001.
