SGS FSMA Self-Assessment Questionnaire

The SGS FSMA Self Assessment questionnaire helps you identify immediate gaps in your system. It covers the Preventive Controls for Human Food rule. The last section of the questionnaire also covers Food Defense. You will be asked if you'd like to complete the Food Defense section before you proceed. Once you complete the self-assessment, you will receive a copy of the completed questionnaire within three business days.
