DPH-SAPC Training Presentation Request Form

Welcome, and thank you for your interest in requesting a DPH-SAPC training presentation!

Please note:

During periods of high demand, staff availability may be limited.
Training requests require at least two weeks' advanced notice.
Regular training hours are Monday–Friday, 8:00 AM–5:00 PM. We will consider requests outside this time frame on a case-by-case basis.
A minimum of 15 participants are required.
Please allow 3 business days for a response.
