Introduction

Why are we doing this survey?

Thank you already for getting started on this with us. We want to know more about what is important to patients when they think about their family doctors, nurse practitioners, or primary healthcare teams (providers). This will help us focus on what really matters to patients when trying to keep track of how well primary care is working.

We have to admit this is a long survey. It isn’t that easy either. It will make you think hard. So we want you to know we really need your help and really will use your input. This is actually the second time we have done this. The first time, we used the data to come up with a better way to measure quality in primary care. We found out that better primary care ends up costing the OVERALL healthcare system less. We hope to find out even more exciting things with the data you give us in this survey. So thank you in advance for taking the time to do this with us.
How does this survey work?

This survey has a very long list of things that health care providers do. Your team or provider may do them or they may not. You might not even know if they do. That’s ok. You might not even have a provider or primary care team. That’s not ok of course, but please stay with us on this survey. We want to know how these things affect how you feel about your team or provider (or WOULD feel, if you had one!).

Here’s an example: Maybe your team has really nice front desk staff. That might make you really feel like your provider or team is sensitive to your feelings. But it might not really affect how smart you think they are. So you would say “very much” for “how sensitive they are to your feelings," but you would say "not at all" for "how smart you think they are." But what if your team does NOT have very nice front desk staff? You can still answer the question. The fact that the staff are NOT nice might make you feel like your provider or team are NOT very sensitive. It still might not affect how smart you think they are. So you would still say “very much” for "how sensitive they are to your feelings,"  and "not at all" for "how smart you think they are." And if how nice the office staff are affects other things (like how trustworthy you think your team is), you can say that too. You can (and probably do) feel more than one thing at a time.

The survey also asks a few questions about you, your health and where you live. We need this to get a sense of how well the results reflect people in general in your province, even though not everyone will answer the survey. The survey takes about 20-25 minutes to complete. Partially completed surveys cannot be saved, so please make sure you leave plenty of time to finish.

You do not have to complete this survey. We hope you do – and if you choose not to, that’s okay – and nobody will know if you did or did not complete it. Your responses are anonymous -- there is no way anyone can ever find out if or how you responded.
