EIT Application for Providers
This is an open application for the St. Louis Area SPOE (Regions 1 & 2) to collect information from providers who are interested in being on an Early Intervention Team in either St. Louis City, St. Louis County, or St. Charles County. Completing this online application is not a guarantee that you will be assigned to a team. As needs arise, the SPOE will access these applications to consider filling the needs of the existing teams.

By completing this application you are agreeing to submit yourself as an individual provider (not to be confused with "independent provider") to be considered for assignment to an Early Intervention Team. If you are employed by an agency, you are solely responsible for notifying your agency administrator(s) that you are applying to be part of a team. Selection and participation for an EIT in no way impacts or changes the status of your enrollment with First Steps (either independent or as an agency provider). Assignment to an EIT DOES NOT change your enrollment to an independent provider. Spots on an EI Team are assigned to the provider selected and not to an agency so the spot belongs to the person. If that individual person leaves the EIT, the spot is open and the SPOE will select the replacement.

* 16. EARLY INTERVENTION TEAM (EIT) CRITERIA


Nothing included in this document is meant to guarantee a position on an EIT for any period of time. Providers who are offered a position on an EIT are expected to provide supports to families in all areas of the designated EIT zone. The SPOE reserves the right to change designated zones based on need. They are also expected to regularly attend IFSP meetings and monthly EIT meetings. If a provider does not meet the criterion indicated in this document at any time, he/she may be replaced.

This criteria notification may be unilaterally changed, altered or amended at any time by AWS/First Steps. Although this notification provides a structure for expectations of EIT providers, AWS /First Steps reserves the right to reconsider the EIT position for reasons including, but not limited to, the following: provider's overall performance, provider reviews, and compliance with First Steps philosophy.


1 Writes Quality DAYC Reports

2 Completes and submits all DAYC and assessment reports and protocols by the time specified on the provider service request form

3 Completes Provider Progress Notes at least monthly

4 Attends all IFSP meetings either in person, by report, or via conference call

5 Contributes to all IFSP meetings by discussing and emphasizing the importance of family routines as well as offering family friendly strategies related to and achievable in the natural environment

6 Attends and contributes to monthly EIT meetings either in person, by report, or via conference call

7 Provides supports as authorized in IFSP

8 Has no substantiated complaints from families, providers, other professionals, or service coordinators regarding work performance and\or professional conduct

9 Is willing to travel and cover all areas of the designated EIT zone

10 Individualizes activities for each family based on their personal concerns

11 Is willing to serve as primary provider and/or joint visit provider as decided upon by the IFSP team

12 Understands and implements the EIT coaching philosophy

13 Attends all SPOE required trainings
