North Shore Parent Survey
WestEd Center for Prevention and Early Intervention (CPEI) has been contracted by the North Shore School District 112 (NSSD 112) to conduct a review of their special education programs and services to identify strengths and provide recommendations for any areas of improvement. WestEd staff will conduct a comprehensive data gathering process and develop a final report with recommendations to be delivered to the Board. WestEd staff will gain a variety of perspectives from multiple data sources including: achievement data, fiscal analysis, document reviews, online surveys and interviews with administrators, educators and staff, focus groups with educators and parents/family members and school site visits at a sampling of sites. The final report will provide a comprehensive review as seen through varied perspectives providing a well-balanced and documented review of NSSD’s special education services.
Surveys are one way that parents and family members will be able to provide feedback on the special education services their students have received. We ask that you complete one survey for each child you have who is receiving special education services.
All of your responses will be kept confidential. Survey results will be returned directly to WestEd; NSSD112 will not see any individual responses or data. The information gathered from the survey will be helpful in verifying the effectiveness of NSSD 112 special education services and in identifying areas for improvement. The survey will take approximately 10-15 minutes to complete. Please complete and return by April 15, 2015.
