Survey Instructions

PLEASE READ THESE DIRECTIONS BEFORE COMPLETING THE SURVEY.

Thank you for facilitating a national conversation, as part of the National Dialogue on Catholic Pastoral Ministry with Youth and Young Adults. The purpose of this survey is for you to summarize the small group conversation you had with either youth, young adults, parents, or ministry leaders. Please note, this survey is for those who used the Synod Questions from the National Dialogue. If you used either the Basic or V Encuentro question set, please return to the National Dialogue webpage and click the appropriate survey link.

While you may have specific opinions or experiences that inform your understanding of youth and young adult ministry, for the purpose of this survey, please only include things related to the conversation that you facilitated. The final page of the survey invites you to share your personal insights gained from the conversation.

  • If you led multiple conversations with different groups, you will need to complete this form for each of the conversations.
  • Question 9 asks you to identify the group with whom you had a conversation based upon the audience type and faith engagement level you are reporting on (disaffiliated, former or marginal Catholic youth and young adults, engaged youth and young adults, parents, or ministry leaders.) If you had a mixed group with both youth and young adults, either select the group that made up the majority of the group you led OR complete the survey twice, once selecting youth and responding to those set of questions and once selecting young adult and responding to those questions. For the purpose of this dialogue, youth includes middle school to high school youth, young adults include those beyond high school.
  • Your response to that question 9, will direct you to a page with the list of the possible conversation questions for this audience so that you can share what you heard. If you did not discuss a particular question, please just skip it. There are up to FIVE text boxes per questions for you to type in ideas, themes, or concepts based upon what you heard. Please make these comments concise and to the point.
