Duxbury FACTS, a coalition to reduce substance use and underage drinking while promoting mental wellness, is surveying Duxbury parents to learn more about their beliefs and perceptions regarding underage drinking and substance abuse. The combined results will be shared with you throughout the year via health promotion activities and a communication campaign.
This 26 question survey is for parents of high school students. When completing these questions, please answer based on your thoughts about your high school child(ren).
This survey is anonymous. Duxbury FACTS thanks the Duxbury Public Schools for making the survey link available to parents on the X2/Aspen portal. Responses will be complied by Survey Monkey and cannot be linked to individual respondents. Duxbury FACTS will ONLY have access to the compiled data. Individual responses will not be part of the report. No one will know how you answered the items in this survey. Please answer the questions based on what you actually think and do.
