Thank you for registering for DSAMT's 10th annual Caleb Thompson Memorial Golf Tournament, presented by North America Administrators and held at the exclusive Five Oaks Golf & Country Club on June 6, 2016.

All players will receive golf at Five Oaks Golf & Country Club, range balls, executive goodie bag, entries for door prizes, beverages on the course, breakfast and lunch and prizes following play.

We encourage each player to secure sponsorships and/or fundraise by asking friends, family and colleagues to make a gift to help enhance the lives of those with Down syndrome.
