You are being asked to participate in the research project described below. Your participation in this study is entirely voluntary and you may refuse to participate, or you may decide to stop your participation at any time. Should you refuse to participate in the study or should you withdraw your consent and stop participation in the study, your decision will involve no penalty. You are being asked to read the information below carefully, and ask questions about anything you don’t understand before deciding whether or not to participate.

Title: Bottlenose dolphin activity in upper Galveston Bay

The purpose of this study is to understand how dolphin activity in upper Galveston Bay has changed in the last few decades. You are invited to complete an online questionnaire that will ask about your perception of dolphins in upper Galveston Bay (e.g. how often and where you have seen them). If you agree to participate, you will be asked a maximum of 34 questions that will take you approximately 5 to 15 minutes to complete. There is no financial compensation for participating in the study. There is also no direct benefit received from your participation, but your participation will help the investigators better understand the ecology of dolphins in upper Galveston Bay.

During actual Internet communication procedures or accessing an unprotected wireless system, there is a possible risk of breach of confidentiality or data security. Every effort will be made to maintain the confidentiality of your study records. Your confidentiality will be kept to the degree permitted by the technology being used (SurveyMonkey). Each subject will be assigned an identification number by the SurveyMonkey administrator, through which all individual’s data will be linked. Computer IP address will not be collected. Your participation may be anonymous, or should you choose, you can provide your contact information at the end of the survey. No guarantees can be made regarding the interception of data via the Internet or email. The data collected from the study will be used for educational and publication purposes, however, you will not be identified by name. Participant’s data for this research project will be maintained and safeguarded on a password-protected database by the Faculty Sponsor for a minimum of three years after completion of the study. After that time, participation documentation may be destroyed.
