Neighborhood & Health Survey
Purpose
We are interested in learning more about your neighborhood conditions and your health. This information will be used to provide better health services and programs for DeKalb County. For this reason, it is important that you answer each question honestly and to the best of your knowledge. We value your thoughts and honest responses. There are no right or wrong answers. We encourage you to answer all of the questions in the survey.
Eligibility
We invite people to participate in this survey who:

Eligibility

- are 18 years of age or older, AND
- live in DeKalb County, AND
- have not yet taken this survey in 2017
Confidentiality
This survey does not ask for your name or any contact information. All information you provide will be kept strictly confidential. Your responses will be combined with the responses of others and you will not be identified in any reports. Your participation in this survey is completely voluntary. You do not have to take the survey, and you may stop the survey at any time.
Incentive
You will have the option to enter a raffle drawing for a chance to win a prize after completing the survey. Prizes will be awarded after all surveys are collected.
Survey Procedures
We will be collecting surveys at community locations throughout DeKalb County during spring/summer 2017. The survey will only take about 10 minutes to complete. Please complete the survey by yourself, but feel free to ask staff if you have any questions.
