Thank you for considering participating in our Collaborative! This community of learning is designed to facilitate local improvement efforts among Ryan White HIV/AIDS Program (RWHAP)-funded providers. The Collaborative focuses on increasing viral suppression performance rates within specific subpopulations of people with HIV/AIDS (PWH) that face social determinants of health barriers. Four Affinity Groups (Housing, Substance Use, Mental Health, and Across the Lifespan) will be set up to accommodate up to 25 RWHAP recipients/subrecipients per group.

Please understand that by applying to participate in this Collaborative, CQII will review all applications and make a determination - in concert with HRSA - to fill all available spots by ensuring a broad representation across various criteria, including HRSA regions, QI competency levels, current performance levels, ability to report performance data, ability to complete all activities, etc.
