Introduction

This survey is designed by the Community Health Worker Advisory Group to collect  baseline information on the prevalence, type and utilization of Community Health Workers (CHWs) in Virginia. 

For the purposes of this survey, a Community Health Worker (CHW) is defined as a trusted member of their community who is trained to provide social support and health-related social needs. A CHW serves as a liaison between community members and health and social services providers. CHWs can be called many different things. Some common examples are: Peer Health Educators, Outreach Workers, Resource Mothers, Health Coaches, Health Advocates, Peer Breast Feeding Counselors, Community Advocates, Health Extension Workers, and Patient Navigators.
