The Center for Health Statistics (CHS) uses this survey to capture information about changes to existing research projects.
Use this form to notify CHS about changes in research staff, data partners, dates of the study, or if you are requesting additional variables or data years. If you want to use data obtained from CHS on a different research project, you must submit a new Data Use Agreement for that project.
Once you have answered the following questions, print and sign the signature form at the end of this survey. Email, fax, or drop off the signed form to Jennifer Woodward, State Registrar, Center for Health Statistics. Contact information is provided at the end of this survey.
