Critical Perspective Headed Into Appropriations Advocacy, 340B Rebate Model, & Coverage Changes


Introduction
State/Regional Primary Care Associations (S/RPCAs), the National PCA Hub, the National Association of Community Health Centers (NACHC), and Advocates for Community Health (ACH) are collaborating to conduct this survey. The survey aims to gain critical perspectives from health centers for use in upcoming advocacy, including to support FY27 appropriations and understand impacts on health centers from H.R. 1 coverage changes and the 340B rebate model. This survey will be open through October 16th.

The survey is broken into four sections: background information about your organization, information about your patients' health insurance coverage, information about your organization's financial status, information about impacts to your services, sites, and workforce, and information about your pharmacy programs.

What To Expect
Questions marked with an * are required; completing as many questions as possible including those that are optional will be helpful and appreciated. You may receive follow-up requests via email based on your survey response (if you choose to share your contact information, but you are welcome to complete the survey anonymously).

You may receive this survey in more than one way (from a PCA, NACHC or ACH, in an email, during a meeting etc.), but you only need to complete it once. Please coordinate inside your organization to only provide one response from your health center.

In 2027, PCAs, NACHC, and ACH will subsequently conduct other surveys to monitor the impact of H.R.1 coverage changes and other factors affecting health centers.

Protection and Use
Aggregate survey information (at state/territory, regional, and national levels, not identifying any single health center) will be used to support advocacy with Congress, the Administration, and other stakeholders. By participating in this survey, you consent to the information in your survey response being accessible to your PCA(s), NACHC and ACH and used as part of aggregate data for advocacy. (State and regional PCAs will receive the survey responses from their respective state(s)/territories. The PCA Hub, NACHC, and ACH will receive all states and territories.)

Individual PCAs, the PCA Hub, NACHC, and ACH will not cite, use, or share individual health center information without a health center's written permission.
