Community Bridges (CB) is an organization in Montgomery County, MD that empowers girls from under-resourced communities to become resilient young women who thrive as students, engaged community members, and confident leaders. We offer a school-based empowerment program to girls in 4th through 12th grades. This form must be completed by a parent or guardian for the student to participate in Community Bridges including school programs, field trips, and events.
Community Bridges collects this information to manage our programs and meet funding requirements. All data is confidential and used for reporting only. Because we are funded to serve low to moderate income families, proof of household income is required to participate. Please complete every section:

Activity Fee: Please note that we have a sliding scale Activity Fee based on family income. If this is financial burden to your family, you pay note how much you are able to pay at the end.
