Informed Consent for Anonymous Online Survey on
Cannabis Use

Dear Participant,
My name is Dr. Karen Yescavage; I am a researcher from Colorado State University – Pueblo in the Psychology department. I am the Principal Investigator conducting a research study on cannabis use (THC and/or CBD) to try to manage psychological pain due to things like stress, anger, anxiety, and PTSD. The purpose is to assess how cannabis impacts (for better and/or worse) negative thoughts and emotions. The title of this project is Cannabis Use and Psychological Pain. This research is being funded by the Institute of Cannabis Research (ICR) at CSU-Pueblo. The purpose of the ICR is neither to endorse nor condemn cannabis use. The purpose of this research is to add to our basic understanding of how and why cannabis is being used and its impact on psychological pain. There are also a few questions regarding social perceptions associated with cannabis use.

If you are 18 years of age or older, I would like you to take an anonymous online survey. It will take approximately 20 minutes for you to complete this survey. Your participation in this research is voluntary. If you decide to participate, you may withdraw your consent and stop participation at any time without penalty.

I will not collect your name or personal identifiers such as IP address. When reporting and sharing the data with others, I will combine the data from all participants. While there are no direct benefits to you, I hope to apply what I learn from you to help future individuals understand to what extent people are using cannabis (THC and/or CBD) who aim to use cannabis (THC and/or CBD) to manage psychological pain in the form of negative emotions and/or thoughts.

There are no known risks to participating in this anonymous survey. It is not possible to identify all potential risks in research procedures, but the researcher(s) have taken reasonable safeguards to minimize any known and potential (but unknown) risks.

To indicate your consent to participate in this research, please continue to the survey. If you do not consent to participate, please exit the survey now. If you are not 18 years or older, you may not participate in this survey; please exit the survey now.

If you have any questions about the research, please contact Dr. Karen Yescavage at If you have any questions about your rights as a volunteer in this research, contact Dr. George Dallam, the CSU-Pueblo IRB chairperson at: or (719) 549-2619.
