Survey on Needs of People at High Risk for Breast, Endometrial, Fallopian Tube or Ovarian Cancer Due to an Inherited Mutation or Family History

Facing Our Risk of Cancer Empowered (FORCE) is a national nonprofit organization for people and families affected by hereditary cancer. We are conducting a survey to better understand the unmet needs of people who are at high risk for breast or gynecologic cancer due to an inherited mutation. We will use the results from this survey to develop new resources and advocate for better medical options to improve outcomes for our community. We will share the combined results of this survey with the community, but your personal information will never be shared. At the end of the survey, you will be given the opportunity to share your contact information for additional follow-up if you choose, but sharing your personal contact information is not required to participate. This survey is voluntary, you may exit the survey at any time. 

This survey should take about 15 minutes to complete.
