Before you complete the registration below, please read the following outline of the project and expectations of participants.
Project Goal
To improve asthma care processes in primary care practices.
Project Duration
The project will begin in April and end in December. A final chart review will be conducted in June 2018 to assess how well you've sustained your improvements.
Goals for Participating Practices
You will use quality improvement tools and methods to improve your ability to:
- Design and implement office policies, processes, and systems that improve asthma care delivery
- Enhance communication and collaboration with specialists
- Utilize resources of the UDOH Asthma Program and the Medical Home Portal to improve asthma care
- Develop knowledge and skills to support ongoing practice improvement
Project Expectations
- Identify your practice’s Improvement Team (clinician leader, office manager, a nurse or medical assistant, more members are welcome)
- Schedule and actively participate in regular meetings (team huddles) with your improvement team (weekly or bi-weekly)
- Participate in site visits with the UPIQ Coach and in monthly virtual team lead calls and educational webinars (4 core webinars and at least one supplementary webinar)
- Complete Pre- and Post-Collaborative Participant Questionnaires
- Complete baseline and project-end (30) and monthly interim (10) chart reviews to guide plan-do-study-act (PDSA) cycles and assess how well your changes are driving improvement
- Use QI TeamSpace (a web-based data collection and QI platform) to record data from monthly chart reviews and track your team's progress
- Develop a plan for sustaining, implementing and spreading your improvements by December
