Dear Participant,

We are conducting a survey study that explores same sex/single male intended parents' experiences with assisted reproductive technology.  

Your participation in this survey study is completely voluntary. There will be no negative consequences should you decide not to participate.

To participate in the survey, you must meet the following criteria:
  1. You must have direct experience (past or current) using third party reproduction in Canada  (i.e. egg donation, and surrogacy)
  2. You must have pursued ART as a single man or an individual within a same sex partnership.

In the survey, you will NOT be asked to provide your name or any personal information that could be used to identify you. In addition, the online survey tool SurveyMonkey will not track your current location, such as your IP address.  All the survey data will be stored on servers in the United States and therefore subject to the PATRIOT Act (https://www.congress.gov/bill/109th-congress/house-bill/03199).  Almost all the questions will ask you to click either on a choice of categories or on a sliding scale. All questions are optional and can be skipped if you do not wish to answer them. Text boxes for open-ended questions are provided so that you can express your views in your own words.  You can skip all open-ended questions if you do not wish to answer any of them. Since this is an anonymous online survey, we hope that you can respond to the questions as honestly and thoroughly as possible.  It is estimated that the survey will take approximately 20-30 minutes to complete.

There is no direct benefit for you to participate in this survey study. This study aims to collect useful data to better understand the motivations, views, expectations, and perspectives of single men and same-sex male couples using assisted reproductive services in order to have children. Your participation may help improve clinical care for this very important demographic group.

There is no anticipated risk as a result of participating in this survey study. However, there is a possibility that some questions in the survey may trigger some unpleasant or negative thoughts you might have regarding your pursuit of having children. If you wish, you may contact our counselling team counselling@createivf.com at the CReATe Fertility Centre for a consultation or utilize counselling resources in your community.

You have the right to terminate the survey at any time simply by exiting the Internet browser. No survey responses will be recorded until you press the “submit” button at the end of the survey. As the data are collected anonymously, you will no longer be able to withdraw your survey responses once you click the “submit” button.
You can email the research team at studycoordinator@createivf.com if you want to be notified about the findings of the study once the results are available. Please use Google or Yahoo to create an anonymous email address if you do not want to use your personal email for contact purposes.

The study you are participating in may be reviewed for quality assurance to make sure that the required laws and guidelines are followed. If chosen, (a) representative(s) of the Human Research Ethics Program (HREP) may access study-related data and/or consent materials as part of the review. All information accessed by the HREP will be upheld to the same level of confidentiality that has been kept by the research team.
